Comparison of Heat-stable Carbetocin and Oxytocin during LSCS and its Relation to Uterine Contractility, Intraoperative Blood Loss, and Patient Hemodynamics in a Tertiary Care Hospital in India

Abstract

Introduction: Lower segment cesarean section (LSCS) is a very common procedure. There has been a lot of research to find the best uterotonic drug, but each drug has its own drawbacks. We aimed to study the efficacy of carbetocin, the new synthetic analog of oxytocin, over the gold standard, oxytocin. Materials and methods: We undertook a prospective randomized controlled study, with 100 women in each group. One group received 100 µg of heat-stable carbetocin IV, and the other 10 IU of oxytocin IM, according to the institutional protocol, within 60 seconds after extraction of the fetus. Results: The use of heat-stable carbetocin was associated with significantly less blood loss during the intraoperative period and better uterine contractility. The hemodynamic parameters of the patients did not show any significant variation following carbetocin use. Conclusion: Carbetocin is very effective for the prevention of atonic postpartum hemorrhage (PPH) and can be used as the drug of choice. It can be stored at room temperature, whereas oxytocin requires an intact cold chain.
