Introduction: Endometriosis has increased in the incidence and prevalence over the last decade. However, incidence of endometriosis in extrapelvic site is quite rare. Cervical endometriosis is one such diagnosis which is rarely encountered in clinical practice, despite being in proximity to the uterus. Cervical endometriosis can have a varied presentation, varying between asymptomatic and severe life-threatening torrential bleed. Case presentation: This article is a case report of a rare case of chronic cervical endometriosis that was successfully managed by surgical intervention. However, the initial clinical examination was suspicious of endocervical malignancy, detailed evaluation revealed the diagnosis of cervical endometriosis. Conclusion: With the rising incidence of atypical presentations of endometriosis, diagnosis of the same poses a great challenge. Diagnostic dilemmas in such cases could be solved with familiarity in the radiological spectrum along with clinical knowledge to arrive at accurate and prompt patient management.
